First, look. Is anybody hurt? If so, call 911 or stop somebody who can do so. Report the accident! Second, get the
name/address of the license of every driver as well as the registration number of the vehicles. Within 10 days after the
accident, ensure a written report of the accident (State Form 52441 – Indiana Operator's Proof of Insurance Crash Report
SR21) goes to the Indiana Bureau of Motor Vehicles. Third, cooperate with authorities but be very careful of any statements
made to anyone except your own insurance company and your attorney.
